Immunizations

Immunization Requirements

All students will need the following immunizations to start school:

4 DTP or DTaP (Diphtheria/Tetanus/Pertussis) with one vaccine on or after the 4th birthday

4 Polio with one vaccine on or after the 4th birthday

2 MMR (Measles/Mumps/Rubella)

3 Hepatitis B

2 Varicella (Chicken Pox) Vaccines or Documentation (Year & Age) of Disease

Grade 7 Only:

1 Tdap (Tetanus/Diphtheria/Acellular Pertussis)

1 MVC (Meningococcal Vaccine)

Grade 12 Only:

1 MVC (Meningococcal Vaccine) given at age 16 or older

*These requirements allow for the following exemptions: medical reasons, religious belief, or philosophical/strong moral or ethical conviction. Please talk to the School Nurse if your child requires an exemption.
