    As we look toward the 2018 spring sports season, there is information necessary for every student athlete involved in participating for Bermudian Springs.

     

    -The PIAA Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) is the only form accepted for participation in the Bermudian Springs athletic program. So if you plan to see your primary care physician, please take the form with you for the necessary signatures and date.  Please use the form that has 2017-18 on the top.  These forms can be found in the athletic physicals section.

     

    -Athletic physicals may not be performed prior to June 1, 2017 to be valid for the 2017-2018 school year.

     

    -If you have a sports physical on file in the athletic office, dated from June 1, 2017 through January 21, 2018, you will need to complete a blue re-certification form to update your health history.  Those forms are for changes in address, insurance, etc. as well as the athlete's health history since his/her physical. Re-certification forms are due to Mrs. Fry at the high school or Mrs. Kohler at the middle school by February 6, 2018

     

    -Those forms will be reviewed and anyone with changes in his/her health history will see our sports medicine physician for a re-certification on February 13, 2018

     

    -Should the re-certification form not be returned by the due date it shall become the athlete's responsibility to pursue re-certification through one's own physician.  A physician's signature is required for re-certification only if there is a change in the health history.

     

    -There are many signatures that are needed for the forms to be considered complete. Please make sure that all signatures are completed for both parents (9 signatures) and student athletes (3 signatures).

     

    -Physicians must sign, date and check the necessary box for clearance for the physical to be valid.

     

    -Insurance company and policy, group or ID information is needed on the form to be considered complete. If you need to purchase school insurance, it can be purchased from the administration office.

     

      -All athletes will need to have a valid ImPACT baseline score on record. This needs to be updated every other year and will take place during practices.
