     As we look toward the 2019-2020 winter sports season, there is information necessary for every student athlete involved in participating for Bermudian Springs.

     -The PIAA Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) is the only form accepted for participation in the Bermudian Springs athletic program. So if you plan to see your primary care physician, please take the form with you for the necessary signatures and date.  Please use the form that has 2019-2020 on the top.  

     -Athletic physicals may not have been performed prior to June 1, 2019 to be valid for the 2019-2020 school year. 

     -There are many signatures that are needed for the forms to be considered complete. Please make sure that all signatures are completed for both parents (9 signatures) and student athletes (3 signatures).

     -Physicians must sign, date and check the necessary box for clearance for the physical to be valid.

     -Insurance company and policy, group or ID information is needed on the form to be considered complete. If you need to purchase school insurance, it can be purchased from the administration office.

    -Winter sport athletes, athletic physicals dated June 1st through October 6, 2019 on record in the athletic office, will need to complete an updated health history.  This is the blue, section 7 recertification form that can be found in the athletic trainer/physicals portion of the website.  These forms are due by October 29th.  If there are changes in the health history, our athletic team physician will evaluate those athletes on November 5th. Anyone missing the deadline, any required recertification will become the responsibility of the athlete. 

    -Wrestling assessments will take place on November 5th.  No assessments will be completed without having the proper paperwork on record.  A second assessment date will be made once the first assessment is completed.  No wrestler may start practice without successfully completing the assessment.

     

     PLEASE DO NOT EMAIL YOUR PAPERWORK, HAND IN THE PRINTED PAPERWORK!

     

    - All athletes will need to have a valid ImPACT baseline score on record. This needs to be updated every other year and will take place during practices.
